Forms can be dowloaded at right to help while participating in the Elves Program. The forms are in PDF format.
For the Winter Holiday Program, after you have contacted an Elf Leader and expressed your interest in participating as a Shopper Elf, you will receive a copy of the Child Profile & Elf Shopping List. This sheet will provide information about the girl or boy you are sponsoring and will give you a checklist of items to buy.
For the Summer Backpack Program, you can use the checklist to purchase items at any time.

Child Profile &
Shopping Checklist
The top half of this form is completed by the parent(s), one for each child. It provides information on each child's clothing sizes, toy "wishes", etc. This information guides Elf shoppers. The bottom half of the form is an important checklist of items; the things we buy for every girl or boy.
